The Aboriginal Health Care Coordination Supplementary Services (CCSS) Program by the Sydney North Health Network is open to people who:
- Reside in the Northern Sydney region, or are patients of a GP within the region
- Identify as Aboriginal, Torres Strait Islander, or both
- Have a chronic disease (heart disease, lung disease, kidney disease, diabetes, cancer) and are at risk of frequent Emergency Department presentations
- Have a GPMP in place
The program offers care coordination and brokerage services to ATSI people in Northern Sydney.
Referral: Required. GP
Catering to: All ages.
Cost: Free service.
Parent or guardian: Not required.
