About

The Aboriginal Chronic Care Coordinator will help you manage your chronic illnesses such as Diabetes , High/Low blood pressure, Asthma, Heart Failure, Renal disease, Mental Health, and Drug & Alcohol issues. There is a 48 Hour Follow-Up after discharge.
The coordinator can help you make your medical appointments, work closely with Aboriginal community agencies to find strategies to support you and your family, provide written and verbal information about your illness and how you can manage it, and provide physical, social, and emotional support for you and your family.
